Health products fail differently. A hallucinated citation in a marketing tool is embarrassing; in a clinical context it's harm. The engineering problem is building a system that knows the boundary of what it knows and routes to a human before it crosses — reliably, not usually.
The system has to be able to decline. We build explicit abstention paths and escalation rules, then evaluate on the cases where the correct answer is "a person should handle this."
